There are two options for sending in your Reward form to Focus on Energy.
1. You can download and print out an application and then mail it to Focus on Energy. (See address on form)

If equipment was installed in a HOME click here: download Residential application
If equipment was installed in a BUSINESS click here: download Business application

2. You can fill out the application online.
NOTE: You will still need to mail in a copy of the paid invoice within 30 days of installation or fax the paid invoice to 608.249.0339 Attn: HVAC Rewards. A copy of the invoice must be received before the reward application can be processed.
